Typically an area is greyed out if there is no cache to find. This will happen in areas where counties are small and cache density is low. You can still organise an event in one of those counties to claim it.

Parts of Lisbon are grayed out (take a look at my profile stats) and I am pretty sure there a) must be caches to find there and b)that parts belong to the same County that surrounds them.
But of course I can be wrong.
Indeed, this seems to be a border error. There are areas in which the political border and the coastline do not match and as far as I can see there should be no 'island' in the Cadaval municipality (in the north). I remember this problem occurred in the past but I am not sure where.
