Why is an unpublished cache appearing in the challenge moderate lists?

We help moderate the challenge checker exception lists.

Tonight this one appeared:

GC8594J - Bezdrożem 1: WORLD

and as we went to look at it, it was said to be as yet unpublished? Any ideas?

We have just had the weekly message “This is a system message. You are receiving this because you are a member of the Challenge checker moderators group, and there are exceptions that needs voting”, and the potentially unpublished cache is the one which the message is referring to

Perhaps it needs removing from the moderating list??

I've seen this happen when the reviewer accidentally publishes the listing and then immediately pulls it back.

Intersting possibility. I still can’t see the cache page, so it must still be unpublished, and when I asked the question, there were no votes on it (as expected) but now I have just visited the challenge checker exception lists page again, and there is one vote on it! Strange?
I noticed this and noted that despite not being able to read the cache page it could still be voted on
