I have noted that I have a different value for number of countries found in my profile (18) and the top #countries  list (which says 19)
I am aware that having logged several 'locationless' caches back in the day - this sometimes skews checkers which think I have been to the USA when I haven't - is this likely to be where the 19th country comes from ?

Seems like you already answered your question yourself.

On profile stats you have usually enabled filter which excludes these location-less, traveling and weird caches (see Settings -> "Use official ignore list for traveling geocaches " on your profile stats), but I believe this filter is not used in the top lists.

It contains also a link to the bookmark list which is used for this filtering out so you can simply check whether you have found something from this list or not.
Use official ignore list for traveling geocaches does not include locationless
I have that box ticked so it shouldn't include travelling anyway - so that seems irrelevant

However even though I may have answered myself - I was looking for clarification
your answer "but I believe this filter is not used in the top lists" maybe clarification but not conclusive
otherwise there is a mistake and I do not know how to rectify the mistake
There is an easy way to find out. Uncheck the checkbox and see if you will have 19 in the stats after it will generate again.
Right, there are real locationless and the "locationless" virtuals that are really in this list (ex. GCF55A).
For example, for me the numbers in stats and in top lists match, but I know that I did not find any "weird" caches, only the above one, which is falling into a country I already cached in.
If you want conclusive answer, you will have to wait for somebody with more knowledge about the code how are the top lists calculated.
